Student Athlete Forms

All Student Athletes must complete these forms prior to the start of any practice. Athletes will not be allowed to participate until these forms have been received by the Athletic Training Staff.

All students are required to show proof of health insurance prior to the beginning of the academic school year.

Northland College does not carry a secondary policy covering athletic related injuries.  Insurance coverage is the responsibility of the student athlete. 

For those who decide to purchase the student health insurance plan ($439) offered at Northland, please note that this plan excludes an injury/sickness resulting from sport participation. Athletes enrolled in this plan are eligible to purchase an additional intercollegiate sports coverage policy. Please note that this extended policy requires a $400.00 premium in addition to the student health insurance plan. If your son/daughter is covered under a family insurance plan, please contact your insurance carrier to see if your policy has any of the following provisions:

  1. Does the policy contain sports exclusion?
  2. Does the policy require any pre-authorizations?
  3. Is the policy an HMO or PPO plan?

Coverage can be limited if you visit a medical facility outside of your designated area. If your plan requires you to designated a clinic in Ashland, the Main Street Clinic is recommended. Our team Physicians Dr. Andy Matheus and Dr. Kathy Gang are associated with that clinic.

Below are copies of the Student Insurance Policy and enrollment forms available through Northland College. Athletes requiring the optional insurance must be enrolled in the Basic Sickness and Accident Policy.

Prior to arriving on campus, please send a copy (front and back) of any health insurance plan the student/athlete may be covered under. Area clinics cannot process health insurance claims without a copy (or original) insurance card.
